It describes how we may use and disclose your health information, our responsibilities, and your rights.
We follow federal law and the health privacy laws of Washington and California. When state law gives you more protection than federal law, we follow the more protective rule.
2. Our responsibilities
We are required by law to:
Maintain the privacy and security of your health information
Give you this notice and follow the terms of the notice currently in effect
Notify you promptly if a breach occurs that may have compromised the privacy or security of your information

If you are unable to agree or object, such as in an emergency, we may share information if we believe it is in your best interest.
A parent, guardian, or other legally authorized representative may exercise your rights on your behalf. Washington and California law allow minors to consent to certain types of care on their own, and in those cases the minor may control access to that information.

8. Your rights
Get a copy of your records. You may ask to see or get an electronic or paper copy of your medical and billing records. We will respond within the time required by Washington or California law, which may be shorter than the federal 30-day limit. We may charge a reasonable, cost-based fee as allowed by law.
Ask us to correct your records. You may ask us to correct information you think is incorrect or incomplete. We may say no, but we will explain why in writing within 60 days, and you may submit a statement of disagreement.
Request confidential communications. You may ask us to contact you in a specific way (for example, a particular phone number or mailing address). We will agree to all reasonable requests.
Ask us to limit what we use or share. You may ask us not to use or share certain information for treatment, payment, or operations. We are not required to agree, except that if you pay for a service in full out of pocket, you may ask us not to share that information with your health insurer, and we will agree unless the law requires the disclosure.
Get a list of disclosures. You may ask for a list of the times we shared your information in the six years before your request, who we shared it with, and why. The list will not include disclosures for treatment, payment, operations, or disclosures you authorized. One list per year is free.
Get a copy of this notice. You may ask for a paper copy at any time, even if you agreed to receive it electronically.
Choose someone to act for you. If someone has your medical power of attorney or is your legal guardian, that person may exercise your rights. We will confirm their authority before acting.
Be notified of a breach. We will notify you as required by law if your unsecured information is affected by a breach.
9. How we communicate with you
We may contact you by phone, voicemail, mail, email, patient portal message, or text message for appointment reminders, scheduling, intake requests, and care follow-up.
